Milton Rents requires all customers to provide an active Certificate of Insurance (COI). This should include the following information:
1. Named Insured
Certificates will only be accepted if the Named Insured matches the individual or business renting the equipment.
2. Commercial General Liability Policy
Occurrence form Each occurrence limit: $1,000,000 General Aggregate $2,000,000
3. Milton CAT & Milton Rents must be listed as Additional Insured on the General Liability Policy
This can be satisfied either by checking the Additional Insured box next to the General Liability policy on the certificate or by including wording that specifically states “Milton CAT & Milton Rents are listed as additional insured with respect to the General Liability policy.”
4. Automobile Liability Policy – for road use vehicles and if customer transports any equipment or uses a 3rd party hauler
Any Auto or Scheduled, Hired, and Non-owned Combined Single Limit: $1,000,000
5. Umbrella/Excess Liability Policy
Occurrence form Each occurrence/Aggregate $1,000,000
6. Equipment Property Coverage
(i.e. Inland Marine, Leased & Rented Equipment, Property Insurance including contractors’ equipment on and off premises, Contractors Equipment Policy, Misc. Equipment) Limit – $100,000 or Replacement Value of the rented equipment, whichever is greater. Coverage to be “All Risks” of direct physical loss Deductible to be no more than $2,500.00 without approval by Milton Rents, Inc.
7. Loss Payee
All Policies must list Milton CAT & Milton Rents as loss payee for leased/rented equipment.
8. Workers Compensation
Statutory Limits for the state where operations are performed with Employers Liability of $500,000 and a waiver of subrogation provided to Milton CAT & Milton Rents where permitted by law.
9. Notice of Cancellation / Non-Renewal
All policies must include endorsements stating the insurance carrier will provide 30 days’ notice of cancellation or non-renewal.
The limits shown are minimum limits and do not limit the customers’ responsibility in the event of a loss.
